Immunization

Connecticut state law requires that you verify immunization against measles, rubella, meningitis and tuberculosis before you enroll at UConn. You and your health care provider need to certify the dates of these immunizations by completing the Health History Form. Please submit the Health History Form included in the Orientation packet you receive in the mail. A copy of the form also is available for downloading and printing .

This confidential record will help us better serve your health needs throughout your academic career at UConn. You must submit your Health History Form by July 15. Students who submit their Health History Form after the July 15 deadline jeopardize their access to University enrollment and housing.
